1.  What problems were you having that caused you to seek treatment in our office?  Contining issues with lower back, neck, and shoulder blade area. 

2.  Please describe how your condition began.  Rough early life (Sports), auto-accident

3.  How long have you had these problems?  Between 15-17 years

4.  Who is your Doctor?  Any comments about that Doctor?  Dr. Schirmer, he has changed my life!

5.  Describe you results and changes that have taken place since you began care.  Reduced headaches, easy sleeping, better mobility and increased energy!

6.  Any additional comments?  I enjoy the energy of the office and their ability to keep me motivated to come back and continue treatment and correct my issues!  Friendly staff, including Kristi and Brittany, make my experience even better!  I've been to many Doctors/Chiropractors, and the HealthFIRST team of Gold's Gym is the BEST by far!!
