1. What problems were you having that caused you to seek treatment in our office?  I have lower back problems.

2.  Please describe how your condition began.  Sharp pain in lower back down to left leg, caused from lifting.

3.  How long have you had these problems?  Almost 2 years.

4.  Please state how these health problems have interfered with your normal life's activities and relationships.  I was out of work and basically immobile for a little over a year.

5. What type of treatment did you seek for these problems prior to Dr. Schirmer?  Epidurals, Cortisone Shots, Physical Therapy, MRI's, Cat Scans, Bone Scans.

6.  Any comments about Dr. Schirmer?  He has changed my life.

7.  Describe your results and/or changes that have taken place since you began care.  I have returned to working full time and I have minimal back pain.

8.  Any additional comments about the advanced techniques used in our office or anything else you would like to say that may help others in their journey to better health?  I personally had a very long, hard road.  I chose chiropractic because I NEEDED a less invasive way to heal myself as opposed to surgery.  I never expected to feel like myself again.  I honestly thought pain was my new reality.  Thanks to Dr. Schirmer's skill and compassion, I can continue to live a healthy and progressive life!
