1.  What problems were you having that caused you to seek treatment in our office?  My left ankle and leg would not heal after 3 months of therapy from another chiropractor.

2.  Please describe how your condition began.  I hurt my ankle at my part time job as a server in a restaurant and ran an 8k the next day. 

3.  How long have you had these problems?  For 3 months before I saw Dr. Schirmer

4.  Please state how these health problems have interfered with your normal life's activities.  It was so hard to run my races, or even to walk.

7.  Any additional comments about the advanced techniques used in our office?  The Foot Levelers has made it so much easier to run!  The staff is excellent!
