1.  What problems were you having that caused you to seek treatment in our office?  Back and neck problems.

2.  Please describe how your condition began.  Horseback riding accident.

3.  How long have you had these problems?  10 years.

4.  Please state how these health problems have interfered with your normal life's activities and relationships.  I was unable to play with my children, I had a hard time lifting objects, I had a hard time working out and I experienced migraines.

5.  How did you find out about our office?  Gold's Gym recommended that I see Dr. Schirmer.

6.  What are you comments about that doctor?  Dr. Schirmer's treatment was the most effective!

7.  Describe your results and/or changes that have taken place since you began care.  I can lift my children and I have very few migraines.

8.  Any additional comments about the advanced techniques used in our office?  Any comments about the treatment, staff, or anything else you would like to say that may help others in their journey to better health?  I love the staff and Doc is AMAZING!  I have not felt this good in many years.  I am thankful that Doc is here and I'm happy to call myself a lifetime chiropractic patient!
