1.  What problems were you having that caused you to seek treatment at our office?  Knee, due to torn meniscus and lower back pain

2.  Please describe how your condition began.  The back pain was due to whiplash and the knee pain was due to a fall. 

3.  How long have you had these problems?  I have had my back problems since 1982.  I have had my knee problems since 2001.

4.  Please state how these health problems have interfered with your normal life's activities and relationships.  I was unable to perform high impact aerobics or running.

5.  How did you find out about our office?  The Personal Trainers in Gold's Gym recommended Dr. Schirmer.

6.  What are your comments about that doctor?  He is truly wonderful!

7.  Describe your results and/or changes that have taken place since you began care.  I was able to resume all physical activities pain-free!

8.  Any additional comments about the advanced techniques used in out office?  I highly recommend the techniques Dr. Schirmer has chosen for me.  I tell everyone about him whenever I'm told someone is experiencing back, neck, or knee problems!
